DSSW's programs are designed to assist persons with disabilities in achieving the greatest degree of independence possible within their community.
The evaluation process by a nurse and the interview process by a program director and your independent case manager assure that an individual’s needs are identified so that they can be met by the services available in each program. CLASS - Community Living Support Services
Community Living Assistance and Support Services (CLASS) provides home and community-based services to people with related conditions as a cost effective alternative to placement in an intermediate care facility for persons with intellectual and developmental disabilities (IDD) or a related condition (ICF-MR/RC).
CLASS provides home and community-based services to people with related conditions as a cost-effective alternative to placement in an intermediate care facility for individuals with an intellectual disability or a related condition. A related condition is a disability, other than an intellectual disability, that originated before age 22 and that affects a person’s ability to function in daily life.

DBMD - Deaf-Blind Multiple Disabilities
Deaf-Blind with Multiple Disabilities (DBMD) provides home and community-based services to people who are deaf-blind with multiple disabilities as a cost effective alternative to institutional placement.

PHC - Primary Home Care
Primary Home Care (PHC) provides non-technical in-home attendant services to people who have an approved medical need for assistance with personal care tasks.” “Client must be 21 or older.
Escort: Accompanying clients on trips to obtain medical diagnosis, treatment, or both. Does not include the direct transportation of the client by the attendant.

CDS - Consumer Directed Services
This service delivery option allows recipients of eligible programs to manage the delivery of a range of services including attendant care. CDS is administered by DSSW’s corporate office. This delivery option gives participants more flexibility in their schedules as well as greater freedom to select attendants and to structure their pay.
